Type specimen: SR97-3-9, a forewing (crumpled & torn fragmentary forewing). Its type locality is Republic, locality B4131, which is in a Ypresian lacustrine - large mudstone in the Klondike Mountain Formation of Washington.

Ecology:

Distribution: found only at Republic, locality B4131
