Type specimen: AMNH 43013. Its type locality is AMNH3026, Windowblind Butte, San Rafael Swell, which is in a Smithian lagoonal/restricted shallow subtidal packstone in the Moenkopi Formation of Utah.

Ecology: epifaunal
