Type specimen: USNM 108494. Its type locality is Walcott Quarry, Fossil Ridge, Greater Phyllopod Bed, which is in a Delamaran deep-water shale in the Burgess Shale Formation of Canada.
Distribution: found only at Walcott Quarry, Fossil Ridge, Greater Phyllopod Bed
