Type specimen: USNM 71713, a forewing (nearly complete forewing). Its type locality is Hermit Shale, Along Bright Angel Trail, which is in an Artinskian/Kungurian fluvial shale/sandstone in the Hermit Shale Formation of Arizona.

Distribution: found only at Hermit Shale, Along Bright Angel Trail
