Type specimen: PV2005.7.2.252.1, a forewing (folded forewing pair). Its type locality is Union Chapel Mine, which is in a Westphalian lagoonal/restricted shallow subtidal shale/sandstone in the Pottsville Formation of Alabama.

Distribution: found only at Union Chapel Mine
