Taxonomy
Phyloblatta levis was named by Schlechtendal (1906). It is an impression. Its type locality is Wettin, which is in a Stephanian C terrestrial shale in the Ottweiler Stufe Formation of Germany.

It was considered a nomen dubium by Duplicate (1910).
