Stellahexaformis was named by Gilmour and Snyder (1986) [Sepkoski's age data: P Guad-u Sepkoski's reference number: 905]. Its type is Stellahexaformis gersterensis.

It was assigned to Dyscritellidae by Gilmour and Snyder (1986); and to Trepostomida by Sepkoski (2002).
