Proliostracus ampliatus was named by Fritz (1991). Its type specimen is GSC 91716, a cephalon/head (cranidium), and it is a mold. Its type locality is Collection 90650, which is in a Dyeran marine limestone/sandstone in the Illtyd Formation of Canada.
