Type specimen: USNM 25631, a wing (fragment). Its type locality is Gibson Fork of Fifteen-Mile Creek, above Decota, which is in a Westphalian B terrestrial siliciclastic in the Kanawha Formation of West Virginia.

Distribution: found only at Gibson Fork of Fifteen-Mile Creek, above Decota
