Type specimen: Brigham Young University Dept of geology no. 3050, a skeleton. Its type locality is Traverse Mountains, Lehi, which is in a Bashkirian marginal marine shale in the Manning Canyon Formation of Utah.

Distribution: found only at Traverse Mountains, Lehi
