Type specimen: USNM 6068, an exoskeleton. Its type locality is Cotton Hill, Fayette County, which is in a Westphalian B terrestrial siliciclastic in the Kanawha Formation of West Virginia.

Distribution: found only at Cotton Hill, Fayette County
