Metoptoma cornutaforme was named by Walcott (1879). It is a 3D body fossil.

It was recombined as Triblidium cornutaforme by Walcott (1912); it was recombined as Proplina cornutaformis by Kobayashi (1933), Knight (1941), Yochelson (1958), Knight and Yochelson (1958), Stinchcomb (1986) and Wagner (2023).
