Type specimen: USNM 531319. Its type locality is SJS-685, McCloud Reservoir, Klamath Mountains, which is in an Asselian/Sakmarian carbonate limestone in the McCloud Formation of California.

Distribution: found only at SJS-685, McCloud Reservoir, Klamath Mountains
