Type specimen: KUVP 150476a,b, a skeleton. Its type locality is Clay Pits, west of Utah Lake, which is in a Bashkirian marginal marine shale in the Manning Canyon Formation of Utah.

Ecology:

Distribution: found only at Clay Pits, west of Utah Lake
