Forcipomyia kulickae was named by Szadziewski (1988). It is an inclusion in amber. Its type locality is Baltic Amber (Museum Ziemi collection), which is in a Priabonian terrestrial amber in Poland.
It was recombined as Forcipomyia (Forcipomyia) kulickae by Borkent and Wirth (1997), Borkent (2013), Borkent and Dominiak (2020).
